How To Apply Master Programme Part (2) - Interview & HLP Meeting

When your application is accepted by HLP, you have to prepare for the interview. 
(It used to be online zoom meeting due to Covid Pandemic)

GUIDE on how to prepare documentations for interview:-
Log in eHLP and click REKOD PERKHIDMATAN 
Print out all the forms at the section Dokumen (red arrow)









These are the documents stated in eHLP and MUST be ready:- 
1. Borang Permohonan 
This is printed version of your HLP application, total 5 pages (page 1 is shown below)













2. Penyokong Pertama 
Your personal detail is prefilled in the form. 














3. Penyokong Kedua 
The form is almost similar as previous one, merely the title changed to "Kedua".
Make sure the letter is sealed in the official envelope "Urusan Seri Paduka Baginda" after written by the referee. Both surat penyokong pertama and pernyokong kedua need to be put in separated envelope. 













4. Pemakluman Temuduga
a) Borang Permohonan Program Kepakaran Sarjana Perubatan Tawaran B 
(as mentioned above)

b) Laporan Penyokong Pertama Dan Kedua 
(as mentioned above)

c) Kenyataan Perkhidmatan yang dikemaskini dan disahkan 
You can retrieve a photocopy of your service book from administration office. 
Normally it takes 1 week for the officer in-charge to print and certify. So, get ready earlier. 













d) Surat Pengesahan Jawatan daripada Suruhanjaya Perkhidmatan Awam (SPA) 
Should you received this letter when you have completed housemanship. 
Or you can ask for a certified photocopy from administration office. 













e) Sijil Pendaftaran Penuh dengan MPM 

f) Laporan SKT 3 tahun yang terkini atau seperti yang dinyatakan 
Again, you need to retrieve a photocopy of the report from administration office. 

g) Dokumen sokongan yang lain (kad pengenalan, sijil SPM, sijil-sijil dan transkrip) 

In Short, 
You have to prepare 3 FILES for interview:- 
i) Portfolio / Personal Profile 
ii) Curriculum Vitae (in my opinion, you can prepare one copy for each interviewer)
iii) Logbook

After The Interview
You shall sit tight, relax and wait for the announcement. 
How to check the result? Yes, by logging in eHLP. 

If you are accepted into the Master Programme, CONGRATULATIONS!

STEPS on accepting the awarded Hadiah Latihan Persekutuan (HLP):-
1. Click the "Berjaya" button (red circle)









2. You will look at the new page "Keputusan Penawaran" 









3. Scroll till the end, choose menerima*, and click "Hantar Pengesahan" button
Beware that there is deadline to accept or reject. 
The timeframe is 7 days from the announcement.
Eg: Result comes out on 30th August, the deadline will be on 5th September.









4. It will turn into "Pengesahan Penerimaan". Click the "Senarai Semak Dokumen".









5. You will read the new page "Senarai Semak Dokumen".
Documents that you have to print out include surat tawaran HLP KKM, slip pengesahan penerimaan tawaran dan borang perjanjian (which i will explain in detail later). Download all the forms. 









6. Go back to main page of eHLP website and click "Dokumen Muat Turun" button (red arrow)









7. Scroll to the section "Tawaran B (Sarjana Perubatan)".
Click "Senarai Semak Dan Kebenaran Mendaftar Di Universiti Awam" form. 
I totally agree the forms are overwhelming. 









8. This is the checklist for HLP meeting / lecture. PREPARE EVERYTHING as it says. 
Reminder, there will be 2 copies written "Salinan KKM" and "Salinan Calon" respectively. 
You have to get ready both copies to get the stamp when you submit your documents. 













These are the documents stated in Senarai Semak Dan Kebenaran Mendaftar Di Universiti Awam and MUST be ready:-
1. Salinan Kad Pengenalan

2. Borang Permohonan (muat turun dari sistem eHLP
(as mentioned above)

3. Surat Tawaran Bersyarat KKM (muat turun dari sistem eHLP)













4. Surat Tawaran Universiti UKM / UM / USM / UNIMAS / UIA / UPM / UITM / UMS 
-sila bulatkan
You will only receive the letter later on when you enrol to respected university. 
You don't have to submit this form during HLP meeting. The university will email to them once available. 

5. Buku Kenyataan Perkhidmatan (yang dikemaskini dan disahkan
(as mentioned above)

6. Borang Pengakuan Dan Pemeriksaan Perubatan (muat turun dari sistem eHLP
-salinan pemeriksaan kesihatan menggunakan borang daripada universiti / klinik / hospital swasta juga boleh diterima
-pegawai diingatkan untuk menyimpan satu salinan untuk simpanan pegawai sendiri
Total 8 pages (page 1 as shown below)

There are few investigations you have to do before examined by the doctor:
a) Biohazard test : Hep B, Hep C, HIV and VDRL 
b) Urine test : UFEME, urine microscopy + urine toxicology 
c) Imaging : Chest X-ray + formal reporting 
Take note of this!
During the university enrolment, they will ask for one more copy of the medical report and test result. 
So, you will have to prepare at least 2 copies (one for HLP meeting, one for university). 













7. Borang Kebenaran Potong Gaji (muat turun dari sistem eHLP)
You can merely fill up the name and idenitification card no and sign. Leave the rest blank. 













8. Salinan Muka Hadapan Buku Akaun Bank / No Akaun Bank 

9. Borang Perjanjian (yang telah disempurnakan) (muat turun dari sistem eHLP)
-3 salinan!
There's guidebook you can refer to prepare this. 
Total 10 pages (page 1 as shown below)

This is most important yet complicated process to prepare.
a) prepare 3 copies 
b) the printing paper must be 80gram and single page 
c) all information is filled by black inked pen and every letter in capital letter 
d) purchase setem hasil (RM10/piece) at any post office and place at top right corner (blue box)
e) get the cop untuk dimatikan at Lembaga Hasil (LHDN)
f) only stapler at specific place (red line)
g) remember to write the date when obtaining the cop (green circle). Valid for 2 months













10. Salinan Perjanjian Ibu Dari JPA (jika berkaitan
-hanya untuk pegawai yang ditaja oleh JPA semasa ijazah perubatan sahaja 

After That,
You will receive letter "Majilis Talimat CBBP HLP KKM Bagi Calon Sarjana Perubatan" via email. 
For Sarawakian and Sabahan, can attend meeting online, then submit the documents using pos laju. 


How To Apply Master Programme Part (1) - HLP KKM CBBP

There are 2 ways to pursue postgraduate studies:- 
A) Hadiah Latihan Persekutuan (HLP) 
B) Parallel Pathway (depends on your interested field availability) 
I'm going to walkthrough for the formal choice. 

WHAT IS CUTI BELAJAR BERGAJI PENUH (CBBP) HADIAH LATIHAN PERSEKUTUAN (HLP)?
(Extracted from buku panduan Taklimat Program Kepakaran Sarjana Perubatan)
"Cuti Belajar" : cuti yang diluluskan oleh Kerajaan kepada seseorang pegawai bagi mengikuti kursus jangka sederhana dan kursus jangka panjang secara sepenuh masa. Dalam tempoh tersebut, pegawai dibenarkan meninggalkan tugas utnuk berkursus dan tidak dikira sebagai putus perkhidmatan atau sebagai berhenti kerja. 

"Biasiswa" : kemudahan yang diperuntukkan oleh Kerajaan Persekutuan, Kerajaan Asing atau Badan Antarabangsa bagi membiayai yuran pengajian, elaun sara hidup dan kemudahan-kemudahan lain semasa dalam tempoh berkursus. 

"Bahagian Pengurusan Latihan (BPL)" : managing unit in-charge with the involved university throughout the Master Programme, that includes ikatan kontrak dan ganti rugi, yuran pengajian, pembayaran elaun-elaun, kelayakan cuti, tindakan tatatertib dan selanjutnya. 
Remember, university is in-charge, not hospital. Kementerian Kesihatan Malaysia (KKM)

"Jawatan Simpanan Latihan (JSL)" : managing unit of pembayaran gaji dan elaun, that includes pergerakan gaji tahunan, kenaikan pangkat, kesan cuti dan perkhidmatan, potongan gaji dan sebagainya. 

GUIDE on management of Bahagian Pengurusan Latihan (BPL) Jawatan Simpanan Latihan (JSL):-
Take note of this! 
1. KELAYAKAN CUTI
a) 14 hari / semester = 28 hari / setahun (permohonan dilaksanakan secara manual, tidak boleh menggunakan Hrmis) 
b) cuti yang berkuatkuasa sebelum Cuti Belajar Bergaji Penuh (CBBP) dijangka luput apabila mula pengajian kecuali 7 days akan disimpan.
c) pegawai tidak layak mengumpul Gantian Cuti Rehat (GCR) apabila ditawarkan ke program pengajian kepakaran sarjana perubatan sejak tahun tarikh tawaran.

2. TUNTUTAN PERPINDAHAN DAN PENGANGKUTAN 
a) hanya layak dibayar pada tahun pertama pengajian (sebelum memulakan kursus) merujuk kepada WPI.3/2013. For this, you will have to deal with our own service hospital. BPL will not handle anything before the course starts. 
b) layak dibayar pada tahun terakhir pengajian selepas melapor diri berjaya dan mendapat penempatan untuk pewartaan. 
c) as of 2022, tidak layak membuat tuntutan pengangkutan kenderaan bagi urusan kargo. 
You are required to submit completed forms to account unit prior transfer. 

These are examples of my hospital application form:-
i) Tuntutan Elaun, Kemudahan Dan Bayaran Bagi Memindahkan Barang-Barang Sebelum / Selepas Berkursus (Lampiran B)

ii) Borang Permohonan Perkhidmatan Pengangkutan Dan Penghantaran Barang-Barang Peribadi Dan Kerera Melalui Jalan Laut 













3. TUNTUTAN GANGGUAN 
(= Pemberian Bayaran Insentif Arahan Pertukaran Wilayah (BIAPW))
a) pegawai diarah bertukar wilayah semenanjung (wilayah asal) ke Sarawak : layak (separuh BIAPW)
b) pegawai bertukar semula ke wilayah asal untuk menyambung pengajian atas permohonan sendiri : tidak layak

4. ELAUN PERKHIDMATAN 
a) Bayaran Intensif Wilayah (BIW) -> BSH (the fare depends on university, eg USM RM300 -> RM250)
b) Elaun Perumahan Wilayah (EPW) -> ITP 

HOW TO APPLY CUTI BELAJAR BERGAJI PENUH (CBBP) DENGAN HADIAH LATIHAN PERSEKUTUAN (HLP)? 
You can ONLY apply online using system eHLP. 
Stay alert to the open application date and submission deadline!
Of course, you will have to fulfil academic requirement as given by respected field before applying.

1. SIGN UP (first timer) or LOG IN (if you already register before).

At the section Alamat Surat Menyurat, can choose Sama Seperti Di Atas*

Be reminded to fill up all the columns as it's easily missed up. (red arrow)
At the section Akademik Tertinggi, choose Sarjana Muda (= Undergraduate)*

At the section Klasifikasi Perkhidmatan, choose Perubatan Dan Kesihatan*
At the section Taraf, the option will be Tetap or Kontrak.

Please pay attention on this! 
You can only obtain LNPT marks from administration office
Apparently the LNPT marks posted online on Hrmis website are different. 
And be aware the specific year of LNPT marks they ask for. 
You can leave cuti blank if you never take any unpaid leave. 


 






9. KEAHLIAN PROFESIONAL 
At the section Bidang, choose Perubatan*


 






10. PANDUAN PERMOHONAN 
You can always refer to their guide book. It's in pdf form. 









11. REKOD PERMOHONAN 
This is most important!!! 
There are 3 types of Tawaran
Tawaran A : Program Sarjana/PHD/Area Of Interest 
Tawaran B : Program Sarjana Perubatan 
Tawaran D : Latihan Kepakaran Parallel Pathway 
For medicine undergraduate who is applying HLP, choose TAWARAN B

Then, fill in the Kursus, Tempat and Tajaan. 
At the section Tajaan, fill in HLP KKM*












Click Hantar Permohonan when you complete. 
Double check if everything is accurate before submission as you can't edit after that. 
Always follow up the Status Penghantaran Permohonan (red circle), that whether 
a) any issue with human resource faculty, 
b) approved by ketua jabatan (head of department or hospital director). 

Once it's all settled, there will be 3 green ticks at the box. And you shall proceed with printing 
a) salinan borang 
b) kegunaan temuduga.

to apply for placement at the hospital you desire
(Sistem Pengurusan Pegawai Perubatan Siswazah).
You ONLY CAN DO IT ONLINE.

INSTRUCTIONS to register and complete the forms on eHouseman
website:-
1. Link to download Manual Pengguna E-Houseman

2. The System eHouseman will be opened EVERY 2 MONTHS.
    The duration of activation is 1 WEEK.
    e.g: this intake on 18 January - 25 January,
           next intake on 7 March - 15 March
    The system always start to activate on 12pm (you can actually
    log in 5-10MIN BEFORE 12pm), hence those wanna compete
    your placement at famous hospitals, please take note!

3. When you receive an email 'PERMOHONAN PENEMPATAN
    BAGI JAWATAN PEGAWAI PERUBATAN GRED UD41 DI
    KEMENTERIAN KESIHATAN MALAYSIA SECARA ATAS
    TALIAN MELALUI SISTEM E-HOUSEMEN' (usually 2-3 days
    before eHouseman opens), you can now open CREATE AN
    ACCOUNT on eHouseman website. (Follow the instructions on
    Manual Pengguna eHouseman for registration of an account)

4. After signing up the account on eHouseman website, you will
    receive an email 'PENDAFTAR SISTEM eHOUSEMEN'. It's
    PEMBERITAHUAN MAKLUMAT PENDAFTARAN SISTEM
    eHOUSEMEN, consisting information of:
    -   Kad Pengenalan
    -   Kata Laluan
    -   Kategori Pegawai
    -   Soalan Keselamatan
    -   Jawapan Keselamatan.
    You CAN ONLY LOG IN to the account when the eHouseman
    opens on the fixed dates.

5. When you log in, the first will be MAKLUMAT PERIBADI.
    The informations that ALREADY FILLED are as below:
    -   Nama
    -   No. Kad Pengenalan
    -   Umur
    -   Tarikh Lahir
    -   Jantina
    -   Agama
    -   Kaum
   The informations that you HAVE TO FILL are:
    -   No. Rujukan SPA (you can retrieve this info from the Surat
        Tawaran at the top right corner, OR you can call to SPA for
        the reference number IN CASE you don't receive your letter)
    -   Alamat (Surat Menyurat), Bandar, Negeri, Poskod
    -   No. Telefon and No. Telefon Bimbit
    -   Emel
    -   Tempat Lahir (Negeri)
    -   Bilangan Adik-Beradik (0,1,2...)
    -   Taraf Perkahwinan
    -   Kewarganegaraan
    -   Bilangan Anak
    -   Pembiayaan (Tiada Biasiswa/JPA/KPM/PTPTN/Lain-Lain
        Biasiswa/Badan Berkanun Kerajaan/Tajaan Swasta/MARA)
    Once done, click 'Seterusnya'.

6. Next will be MAKLUMAT KELUARGA.
    The informations that you HAVE TO FILL are:
    -   Nama
    -   No. Kad Pengenalan
    -   Hubungan
    -   Alamat (Surat Menyurat), Bandar, Negeri, Poskod
    -   No. Telefon Bimbit (jika ada)
    -   Emel (jika ada)
    -   Umur
    -   Agama
    -   Kewarganegaraan
    -   Kaum
    -   Jawatan (jika ada)
    -   Sektor Pekerjaan (Badan Berkanun/Kerajaan/Swasta/Tiada)
    -   Pekerjaan (Perdagangan/Pendidikan/Pembinaan etc)
    Once done, click 'Seterusnya'.

7. Then will be MAKLUMAT PENDIDIKAN.
    The informations that ALREADY FILLED are as below:
    -   Nama
    -   No. Kad Pengenalan (MyKad)
    The informations that you HAVE TO FILL are:
    -   Lain-Lain Kelayakan
    -   Tahun Kelayakan
    The following are THE OPTIONS in Lain-Lain Kelayakan:
    -   A Level
    -   Australian Matriculation Programme (AUSMAT)
    -   Diploma
    -   Diploma/Diploma Lanjutan Kemahiran Malaysia
        (DKM/DLKM)
    -   International Baccalaureate Diploma (IBD)
    -   Lain-Lain
    -   Matrikulasi
    -   O Level
    -   Sekolah Sukan Bandar Penawar
    -   Sekolah Sukan Bukit Jalil

8. Last will be PENEMPATAN/PENANGGUHAN.
    Before you choose your placement, there will be a notice.
    PERHATIAN: SILA BACA DENGAN TELITI
    MAKLUMAT BERIKUT:-
    -   Pegawai wajib membuat SATU pilihan sahaja sama ada
        PENEMPATAN atau PENANGGUHAN.
    -   Hanya hospital latihan siswazah yang mempunyai
        kekosongan sahaja dipaparkan dalam senarai Pilihan
        Penempatan.
    -   Jika pegawai memilih membuat penangguhan, pegawai
        TIDAK AKAN dilantik untuk sesi pengambilan ini.
    -   Pilihan Penempatan di dalam Aplikasi ini adalah
        MUKTAMAD. Slip/Surat Tawaran boleh dicetak
        selepas permohonan penempatan sistem.

    If you want PENEMPATAN, you will have to choose the
    state in Pilihan Negeri, then in Pilihan Penempatan will show
    the name of hospitals and the number of seats left.
    e.g: Pilihan Negeri : Sarawak
           Pilihan Penempatan : Hospital Sibu (97 kekosongan)
                                              Hospital Bintulu (55 kekosongan)
                                              Hospital Umum Sarawak, Kuching
                                              (43 kekosongan)
    If you want PENANGGUHAN, you have to answer TEMPOH
    PENANGGUHAN and ALASAN PENANGGUHAN.
    Once done, click 'Seterusnya'.

9. Proceed to PENGAKUAN.
    Double confirm the hospital you chose is correct. If all is
    okay, tick the box 'Saya mengaku', then click 'Hantar'.
    There will be a pop up window PERMOHONAN BERJAYA.
    Click 'Cetak'.

10. Now, you will receive an email 'PENEMPATAN PEGAWAI
      PERUBATAN GRED UD41 DI KEMENTERIA KESIHATAN
      MALAYSIA'.

AFTER THAT,

Important ERRANDS that needed to complete before attending
INDUCTION PROGRAM (PTM):-
1.    Fill up SURAT SETUJU-TERIMA PELANTIKAN S.P.A.6A
       and post back to SPA within 1 month from the date you
       receive your surat tawaran SPA (they will send you 3 copies,
       you just send ONLY 1 COPY back)
       -   HOW TO FILL UP:
           Please use the info from Surat Tawaran SPA.
           Saya terima tawaran pelantikan sebagai (jawatan dalam
           surat), bilangan SPA.PG (no. rujukan kami), bertarikh
           (tarikh surat bawah no. rujukan kami)
       -   You can send back the letter either by:-
           a. DELIVERY YOURSELF
           b. MAILING (normal post/registered post/pos laju)
       -   Here is the MAILING ADDRESS:

2.    Prepare SURAT AKUAN SUMPAH SPA 6C
       -   Get the cop from Pesuruhanjaya Sumpah/Majistret/
           Hakim Mahkamah Sesyen
       -   The cop maybe FOC or charge RM3-5 per piece
       -   1 piece is enough (but i get 2 pieces just in case)
3.    Do MEDICAL CHECK UP USING BORANG PIN. 1/2013
       -   You can choose any clinic (private/government)
       -   Preferably Klinik Kesihatan (the staffs are familiar with
           the process and procedure, and it's FOC)
       -   You should bring your Surat Tawaran SPA together with
           Borang Pin. 1/2013 for verification
       -   You will get Routine Physical Examination, Chest X-Ray,
           Urine Test for glucose, albumin and DRUGS
       -   Best to get your medical check up done 2-3 WEEKS
           BEFORE PTM (the lab test results might take time)
4.    Complete the E-VETTING (BORANG KELULUSAN
       TAPISAN KESELAMTAN KASAR)

       -   In PENDAFTARAN BARU (when register):-
           Jenis Pendaftaran : PEMOHON INDIVIDU
       -   In PERMOHONAN BARU (after log in):-
           Click 'LANTIKAN SURUHANJAYA PERKHIDMATAN
           AWAM (SPA) ATAU PENTADBIRAN KERAJAAN
           NEGARA ATAU PIHAK BERKUASA TEMPTAN (PBT)'
       -   In MAKLUMAT PROFIL:-
           a. Pick a clear photo of yourself with clean blue background
               and no photobomb (size less than 200kb)
               Tips : scan your passport photo and upload
       -   In MAKLUMAT PERJAWATAN:-
           a. Kementerian/Negeri : KEMENTERIAN KESIHATAN
                                                  MALAYSIA
           b. Jabatan : (your state jabatan kesihatan)

       Pekerja (KWSP)
       -   The only document you need for registration is your IC
           (MyKad).
7.    Register a INCOME TAX ACCOUNT at Lembaga Hasil
       Dalam Negeri Malaysia (LHDN)
       -   Just bring a photocopy of your IC (MyKad) and fill up
           the registration form provided at the counter.
8.    Make your own a SELF-INKING COP
       -   Here is the FORMAT OF COP:
           Your name
           Pegawai Perubatan Gred UD41
           MMC No.: xxxxx
           Your working hospital
9.    Make your own NAME TAG
       -   It should be white wordings with black background
10.  Buy your own LABCOAT (OPTIONAL)
       -   Yes, the hospital will lend you the labcoat, but the size
           will be limited (lack of size S most of the time), and
           personally i don't like to share my clothes
       -   Strongly recommend to get the short sleeves labcoat
           as running around the hospital will be hot and sweaty
11.  Get 8 pieces of PASSPORT SIZE PHOTOS
       -   3 photos are needed for PTM
       -   5 photos are needed for lapor diri at the hospital

LASTLY,

attend PROGRAM TRANSFORMASI MINDA (PTM),
to learn how to apply excellent work culture with first class
mentality and be patriotism (to brainwash you with good).
You MUST PRESENT WITHOUT ANY EXCUSE.

1.    Surat Panggilan Transformasi Minda Sementara
       -   Surat Panggilan sebenar PTM akan diserah kepada
           Tuan/Puan semasa PTM
2.    Lampiran A and C (2 Keping)
3.    Lampiran B and D
       -   Reminders For ATTIRE:
           -   It's OBLIGATORY to wear BAJU BATIK on Thursday
           -   NO round neck T-shirt is allowed
           -   NO short sleeves collar shirt is allowed (for men)
           -   NO bright color pants are allowed
           -   Shoes should be dark color and cover the toes
4.    Gambar Berukuran Passport (3 Keping)
       -   2 pieces for Lampiran A
       -   1 piece used during pendaftaran PTM

DIRI for PENGESAHAN LANTIKAN:-
1.    Tuan/Puan wajib membawa dokumen-dokumen asal dan
       salinan fotostat semasa lapor diri di hospital masing-masing
       kepada pihak urus setia JKN.
2.    Tuan/Puan dikehendaki sedia 3 SALINAN FOTOSTAT
       untuk setiap dokumen yang berikut.
3.    Surat Tawaran Pelantikan SPA
4.    Surat Penempatan KKM
       -   Dimuat turun selepas buat ehousemen
5.    Surat Penempatan JKN
       -   Diserahkan kepada Tuan/Puan selepas tamat PTM
6.    Surat Setuju - Terima Pelantikan S.P.A.6A
7.    Surat Akuan Sumpah SPA 6C (2014)
       -   Membuat Akuan Berkanun di hadapan Pesuruhanjaya
           Sumpah/Majistret/Hakim Mahkamah Sesyen
8.    Surat Akuan Pemeriksaan Perubatan Borang Pin. 1/2013
       -   Menjalani Pemeriksaan Perubatan dan diperakui oleh
           seorang Pegawai Perubatan
       -   Pemeriksaan kesihatan boleh dilakukan di mana-mana
           hospital/klinik kerajaan/klinik swasta
