INSTRUCTIONS to register and complete the forms on eHouseman
website:-
1. Link to download Manual Pengguna E-Houseman

2. The System eHouseman will be opened EVERY 2 MONTHS.
    The duration of activation is 1 WEEK.
    e.g: this intake on 18 January - 25 January,
           next intake on 7 March - 15 March
    The system always start to activate on 12pm (you can actually
    log in 5-10MIN BEFORE 12pm), hence those wanna compete
    your placement at famous hospitals, please take note!

3. When you receive an email 'PERMOHONAN PENEMPATAN
    BAGI JAWATAN PEGAWAI PERUBATAN GRED UD41 DI
    KEMENTERIAN KESIHATAN MALAYSIA SECARA ATAS
    TALIAN MELALUI SISTEM E-HOUSEMEN' (usually 2-3 days
    before eHouseman opens), you can now open CREATE AN
    ACCOUNT on eHouseman website. (Follow the instructions on
    Manual Pengguna eHouseman for registration of an account)

4. After signing up the account on eHouseman website, you will
    receive an email 'PENDAFTAR SISTEM eHOUSEMEN'. It's
    PEMBERITAHUAN MAKLUMAT PENDAFTARAN SISTEM
    eHOUSEMEN, consisting information of:
    -   Kad Pengenalan
    -   Kata Laluan
    -   Kategori Pegawai
    -   Soalan Keselamatan
    -   Jawapan Keselamatan.
    You CAN ONLY LOG IN to the account when the eHouseman
    opens on the fixed dates.

5. When you log in, the first will be MAKLUMAT PERIBADI.
    The informations that ALREADY FILLED are as below:
    -   Nama
    -   No. Kad Pengenalan
    -   Umur
    -   Tarikh Lahir
    -   Jantina
    -   Agama
    -   Kaum
   The informations that you HAVE TO FILL are:
    -   No. Rujukan SPA (you can retrieve this info from the Surat
        Tawaran at the top right corner, OR you can call to SPA for
        the reference number IN CASE you don't receive your letter)
    -   Alamat (Surat Menyurat), Bandar, Negeri, Poskod
    -   No. Telefon and No. Telefon Bimbit
    -   Emel
    -   Tempat Lahir (Negeri)
    -   Bilangan Adik-Beradik (0,1,2...)
    -   Taraf Perkahwinan
    -   Kewarganegaraan
    -   Bilangan Anak
    -   Pembiayaan (Tiada Biasiswa/JPA/KPM/PTPTN/Lain-Lain
        Biasiswa/Badan Berkanun Kerajaan/Tajaan Swasta/MARA)
    Once done, click 'Seterusnya'.

6. Next will be MAKLUMAT KELUARGA.
    The informations that you HAVE TO FILL are:
    -   Nama
    -   No. Kad Pengenalan
    -   Hubungan
    -   Alamat (Surat Menyurat), Bandar, Negeri, Poskod
    -   No. Telefon Bimbit (jika ada)
    -   Emel (jika ada)
    -   Umur
    -   Agama
    -   Kewarganegaraan
    -   Kaum
    -   Jawatan (jika ada)
    -   Sektor Pekerjaan (Badan Berkanun/Kerajaan/Swasta/Tiada)
    -   Pekerjaan (Perdagangan/Pendidikan/Pembinaan etc)
    Once done, click 'Seterusnya'.

7. Then will be MAKLUMAT PENDIDIKAN.
    The informations that ALREADY FILLED are as below:
    -   Nama
    -   No. Kad Pengenalan (MyKad)
    The informations that you HAVE TO FILL are:
    -   Lain-Lain Kelayakan
    -   Tahun Kelayakan
    The following are THE OPTIONS in Lain-Lain Kelayakan:
    -   A Level
    -   Australian Matriculation Programme (AUSMAT)
    -   Diploma
    -   Diploma/Diploma Lanjutan Kemahiran Malaysia
        (DKM/DLKM)
    -   International Baccalaureate Diploma (IBD)
    -   Lain-Lain
    -   Matrikulasi
    -   O Level
    -   Sekolah Sukan Bandar Penawar
    -   Sekolah Sukan Bukit Jalil

8. Last will be PENEMPATAN/PENANGGUHAN.
    Before you choose your placement, there will be a notice.
    PERHATIAN: SILA BACA DENGAN TELITI
    MAKLUMAT BERIKUT:-
    -   Pegawai wajib membuat SATU pilihan sahaja sama ada
        PENEMPATAN atau PENANGGUHAN.
    -   Hanya hospital latihan siswazah yang mempunyai
        kekosongan sahaja dipaparkan dalam senarai Pilihan
        Penempatan.
    -   Jika pegawai memilih membuat penangguhan, pegawai
        TIDAK AKAN dilantik untuk sesi pengambilan ini.
    -   Pilihan Penempatan di dalam Aplikasi ini adalah
        MUKTAMAD. Slip/Surat Tawaran boleh dicetak
        selepas permohonan penempatan sistem.

    If you want PENEMPATAN, you will have to choose the
    state in Pilihan Negeri, then in Pilihan Penempatan will show
    the name of hospitals and the number of seats left.
    e.g: Pilihan Negeri : Sarawak
           Pilihan Penempatan : Hospital Sibu (97 kekosongan)
                                              Hospital Bintulu (55 kekosongan)
                                              Hospital Umum Sarawak, Kuching
                                              (43 kekosongan)
    If you want PENANGGUHAN, you have to answer TEMPOH
    PENANGGUHAN and ALASAN PENANGGUHAN.
    Once done, click 'Seterusnya'.

9. Proceed to PENGAKUAN.
    Double confirm the hospital you chose is correct. If all is
    okay, tick the box 'Saya mengaku', then click 'Hantar'.
    There will be a pop up window PERMOHONAN BERJAYA.
    Click 'Cetak'.

10. Now, you will receive an email 'PENEMPATAN PEGAWAI
      PERUBATAN GRED UD41 DI KEMENTERIA KESIHATAN
      MALAYSIA'.

AFTER THAT,

Important ERRANDS that needed to complete before attending
INDUCTION PROGRAM (PTM):-
1.    Fill up SURAT SETUJU-TERIMA PELANTIKAN S.P.A.6A
       and post back to SPA within 1 month from the date you
       receive your surat tawaran SPA (they will send you 3 copies,
       you just send ONLY 1 COPY back)
       -   HOW TO FILL UP:
           Please use the info from Surat Tawaran SPA.
           Saya terima tawaran pelantikan sebagai (jawatan dalam
           surat), bilangan SPA.PG (no. rujukan kami), bertarikh
           (tarikh surat bawah no. rujukan kami)
       -   You can send back the letter either by:-
           a. DELIVERY YOURSELF
           b. MAILING (normal post/registered post/pos laju)
       -   Here is the MAILING ADDRESS:

       -   Get the cop from Pesuruhanjaya Sumpah/Majistret/

       -   The cop maybe FOC or charge RM3-5 per piece
       -   1 piece is enough (but i get 2 pieces just in case)
3.    Do MEDICAL CHECK UP USING BORANG PIN. 1/2013
       -   You can choose any clinic (private/government)
       -   Preferably Klinik Kesihatan (the staffs are familiar with
           the process and procedure, and it's FOC)
       -   You should bring your Surat Tawaran SPA together with
           Borang Pin. 1/2013 for verification
       -   You will get Routine Physical Examination, Chest X-Ray,
           Urine Test for glucose, albumin and DRUGS
       -   Best to get your medical check up done 2-3 WEEKS
           BEFORE PTM (the lab test results might take time)
4.    Complete the E-VETTING (BORANG KELULUSAN
       TAPISAN KESELAMTAN KASAR)

               and no photobomb (size less than 200kb)

5.    Set up a PERSONAL BANK ACCOUNT
       -   You can choose any bank you like
       -   Preferably Maybank
       -   Remember, it's your personal saving account, cannot be
           joined names
6.    Register a EPF ACCOUNT at Kumpulan Wang Simpanan
       Pekerja (KWSP)
       -   The only document you need for registration is your IC
           (MyKad).
7.    Register a INCOME TAX ACCOUNT at Lembaga Hasil
       Dalam Negeri Malaysia (LHDN)
       -   Just bring a photocopy of your IC (MyKad) and fill up
           the registration form provided at the counter.
8.    Make your own a SELF-INKING COP
       -   Here is the FORMAT OF COP:
           Your name
           Pegawai Perubatan Gred UD41
           MMC No.: xxxxx
           Your working hospital
9.    Make your own NAME TAG
       -   It should be white wordings with black background
10.  Buy your own LABCOAT (OPTIONAL)
       -   Yes, the hospital will lend you the labcoat, but the size
           will be limited (lack of size S most of the time), and
           personally i don't like to share my clothes
       -   Strongly recommend to get the short sleeves labcoat
           as running around the hospital will be hot and sweaty
11.  Get 8 pieces of PASSPORT SIZE PHOTOS
       -   3 photos are needed for PTM
       -   5 photos are needed for lapor diri at the hospital

LASTLY,

attend PROGRAM TRANSFORMASI MINDA (PTM),
to learn how to apply excellent work culture with first class
mentality and be patriotism (to brainwash you with good).
You MUST PRESENT WITHOUT ANY EXCUSE.

The following documents are needed to prepare to PTM:-
1.    Surat Panggilan Transformasi Minda Sementara
       -   Surat Panggilan sebenar PTM akan diserah kepada
           Tuan/Puan semasa PTM
2.    Lampiran A and C (2 Keping)
3.    Lampiran B and D
       -   Reminders For ATTIRE:
           -   It's OBLIGATORY to wear BAJU BATIK on Thursday
           -   NO round neck T-shirt is allowed
           -   NO short sleeves collar shirt is allowed (for men)
           -   NO bright color pants are allowed
           -   Shoes should be dark color and cover the toes
4.    Gambar Berukuran Passport (3 Keping)
       -   2 pieces for Lampiran A
       -   1 piece used during pendaftaran PTM

       salinan fotostat semasa lapor diri di hospital masing-masing
       kepada pihak urus setia JKN.
2.    Tuan/Puan dikehendaki sedia 3 SALINAN FOTOSTAT
       untuk setiap dokumen yang berikut.

4.    Surat Penempatan KKM
       -   Dimuat turun selepas buat ehousemen

So, we are done with MALAYSIA MEDICAL COUNCIL (MMC) and
KEMENTERIAN KESIHATAN MALAYSIA (KKM).

AND NOW,

go to SELURUHANJAYA PERKHIDMATAN AWAM (SPA),
to register employment in public service SPA8i
(Pendaftaran Pekerjaan Dalam Perkhidmatan Awam).
You ONLY CAN DO IT ONLINE.

INSTRUCTIONS to register and complete the forms on SPA website:-
1. The recommended web browsers are INTERNET EXPLORER and
    MOZILLA FIREFOX. The webpage will crash if you use other
    browsers, trust me.
2. Open the SPA official website. Click on PERMOHON PERKERJAAN.

3. Click on SERVER 1 or SERVER 2 under Pendaftaran Pekerajaan
    Dalam Perkhidmatan Awam (SPA8i).

4. Click on DAFTAR AKAUN BARU at SPA8i Login.

5. Fill up all the details and click 'Daftar'.

6. Log in to SPA8i and fill up MAKLUMAT PEMOHON. If you are
    holding car driving licence, tick 'D'; If you are holding motorcycle
    licence, tick 'B2'. Once done, click 'Simpan'.

7. Click on MAKLUMAT AKADEMIK and fill up 'PMR/SRP/LCE'.
    Once done, click 'Simpan'.

8. Proceed to 'SPM/MCE/SPM(V)'. Actually they already filled up the
    information for you.

9. Proceed to 'Pengajian Tinggi'. Here's the tricky part. The following
    information is for Pegawai Perubatan Gred UD41.
    Peringkat Kelulusan : SARJANA MUDA DLM BDG PROF
    Bidang : PERUBATAN
    Pengkhususan : PERUBATAN DAN PEMBEDAHAN
    cGPA (PNGK) is no needed if your university isn't using this as
    marking system. Once done, click 'Simpan'.

10. Proceed to 'Profesional/Iktisas'. This part can only be done when
      you have your SIJIL PENDAFTARAN SEMENTARA. Choose
      BERDAFTAR DENGAN MAJLIS PERUBATAN MALAYSIA.
      Fill in MMC REGISTRATION NUMBER and DATE OF
      REGISTRATION. Once done, click 'Simpan'. If you add this
      information afterward, remember go to 'Pengakuan Pemohon',
      tick 'Saya Setuju' and click 'Hantar' again.

11. Click on MAKLUMAT KO-KURIKULUM and fill up 'Sukan/
      Persatuan' and/or 'Rekacipta/Pencapaian'. If you don't join any club,
      simply choose 'Tiada Maklumat Ko-kurikulum'.

12. Click on MAKLUMAT TAMBAHAN. Fill up 'Bakat/Kebolehan
      Bahasa' and/or 'Bekas Tentera/Polis' and/or 'Penerima Bantuan/
      Kurang Upaya'. If you don't have anything to add, just choose 'Tiada
      Maklumat Tambahan'.

13. Click on JAWATAN DIPOHON. Find and choose PEGAWAI
      PERUBATAN GRED UD41. Once done, click 'Simpan'.

14. Click on PENGAKUAN PEMOHON. Check every detail if it's
      correct. If yes, tick the box 'Saya Setuju' and click 'Hantar'. Click
      on 'Resume' and PRINT the resume.

15. Click on SEMAKAN PERMOHONAN. There will be 'Tiada
      Maklumat' at this moment.

16. Thus, you have to log in SPA8i from time to time and check on
      SEMAKAN PERMOHONAN. There will be updates when:-
      a) You are called to the SPA INTERVIEW
      b) You receive the SURAT TAWARAN

AND THEN,

You wait for the SPA INTERVIEW.
(Email 'Panggilan Temu Duga Pegawai Perubatan Gred UD41').
They will notify you by EMAIL.
Thus, make sure you give a reachable and appropriate email address
when you fill up SPA8i. And, CHECK YOUR EMAIL occasionally.

The following DOCUMENTS are needed to submit during SPA
INTERVIEW:-
1.   Tuan/Puan dikehendaki membawa dokumen-dokumen asal dan
      salinan fotostat yang disahkan.
2.   Kad Pengenalan
3.   Sijil Kelahiran
4.   Sijil Kewarganegaraan Malaysia atau bukti-bukti lain yang
      mengesahkan Kerakyatan Malaysia seperti Sijil Kewarganegaraan
      Ibu/Bapa (jika berkenaan)
5.   Sijil Kelulusan PMR/SRP
6.   Sijil Kelulusan SPM/SPM(V)
7.   Sijil yang membuktikan kelulusan Bahasa Melayu setaraf
      kelulusan SPM (jika berkenaan)
8.   Sijil Kelulusan STPM/STP atau yang setaraf
9.   Sijil A-Level/Diplomma atau yang setaraf
10. Sijil Ijazah dan Transkrip/Surat Pengesahan Kelulusan Peperiksaan

      fotostat dokumen yang diserahkan TIDAK AKAN dikembalikan

TIPS for SPA INTERVIEW:-
1. To keep your CERTIFICATES ORGANISED
    -   Arrange all your ORIGINAL CERT in a PORTFOLIO FOLDER
        WITH TRANSPARENT POCKETS, in the order based on the
        numbering stated in the email.
    -   Put your CERTIFIED PHOTOSTATE CERT in a CATALOG
        ENVELOPE, in the order according to the sequence you display
        your original cert.
2. To write a SIMPLE AND BRIEF RESUME
    -   Right, you DON'T USE the 'Resume' ready for you in SPA8i.
    -   The resume should be ONLY 1-2 PAGES long.
    -   The resume should be easy to read and eye catching. Use the
        STANDARD FONT (Time New Roman or Cambria, in 12pt size
        are generally acceptable).
    -   The resume SHOULD INCLUDE Personal Information, Career
        Objective, Education/Qualification, Additional Skills (Languages,
        Computer/ICT, Key Skills etc), Achievements/Activities related to
        the pursued job, and Working Experience.
    -   In Personal Information SHOULD NOT INCLUDE details of
        your family, your pet, hobbies, religion, and relationship status.
    -   In Contact Information BEST INCLUDE ONLY 1 email address
        (formal and professional), 1 phone number despite multiple ones.
3. Be PUNCTUAL
    -   Be there 1/2 - 1 HOUR EARLIER than the time fixed to you in
        the email. It never good to let the interviewer waiting for you,
        isn't it? If you reach there earlier, you can double check your
        documents with the staff and calm your nervous state.
4. DRESS APPROPRIATE AND FORMALLY
    -   For WOMEN
        -   Sparse/Natural Makeup
        -   Clean and tidy Hair Updos
        -   Long Sleeved Shirt (preferably white)
        -   Suit/Blazer (preferably black)
        -   Knee-Length Skirt/Long Pants
        -   Leather Pumps Heels (comfortable and walk with ease)
        -   Little/No Jewellery

And now you gonna create your next chapter, what's next?
Yes, it will be HOW TO APPLY HOUSEMANSHIP IN MALAYSIA.

FIRST OF ALL,

Make sure you have your MD/MBBS/MBBoH Certificate.
Translate it if they are written in foreign language.
(this should be done at your university respectively)

SECONDLY,

go to MALAYSIA MEDICAL COUNCIL (MMC),
to apply for the provisional registration section 12 and 13
(SIJIL PENDAFTARAN SEMENTARA).
You ONLY CAN SUBMIT YOURSELF.

1. Get down at Pasar Seni Station (Kelana Jaya/Terminal Putra line).


2. Walk across the bridge and you will find the post office headquarter
    on your right.

3. Walk around the post office until you reach Pusat Penyelidikan
    Bioteknologi Mardi and walk across the bridge.

4. You will find the underground staircase at the left side. When you exit
    from the other side of tunnel, you will see a public toilet on your left.

5. Walk straight along the road. There's a school on your left, and MMC
    on your right (although the sign board shows 'Kementerian Kesihatan
    Malaysia').
6. Ask the guard at the entrance and they will lead you the way.

4.    Borang Pengakuan Pengenalan Saksi
       disahkan oleh Pegawai Gred 41 dan ke atas
       atau Pengamal Perubatan Berdaftar Penuh
5.    Borang Maklumat Tambahan
6.    Salinan Kad Pengenalan yang disahkan
7.    Salinan Sijil Kelahiran yang disahkan
8.    Salinan Sijil Pelajaran Malaysia yang disahkan atau yang setaraf
9.    Salinan Ijazah Asas Perubatan yang disahkan
       atau Surat Asal Dekan
10.  Transkrip Peperiksaan bagi keseluruhan pengajian yang disahkan
11.  Sekiranya dokumen asal bukan dalam BM atau BI:

13. Jika nama anda di dalam ijazah/surat dekan/transkrip dan kad
      pengenalan berbeza, sila kemukakan Surat Akuan Sumpah yang
      menyatakan bahawa individu dengan name tertera pada salinan
      ijazah/surat dekan/transkrip dan kad pengenalan tersebut adalah
      orang yang sama.

You can COLLECT the document either by:-
a. GETTING YOURSELF
b. A REPRESENTATIVE (give the MMC your rep's copy of IC and a
    letter stating you give him/her permission to collect on behalf of you)
In my point of view, i PREFER to pick up the document yourself. You
will need the cert for future purpose. Beside, it's best to avoid any
irrelevant people to know your MMC REGISTRATION NUMBER.

Usually you have to wait for 1 WEEK from the day you hand in the
documents to MMC. The staff will give you a paper written the date
you can return to pick up the SIJIL PENDAFTARAN SEMENTARA.
For non-KL residents, if you wanna pre-book your air ticket, i suggest
you give about 2 WEEKS window. (just in case if there's any delay in
process, you don't have to burn your air ticket)

THIRDLY,

go to KEMENTERIAN KESIHATAN MALAYSIA (KKM),
to apply for deployment as a medical personnel
(Permohonan Penempatan Pegawai Perubatan).

You can SUBMIT the documents either by:-
a. DELIVERY YOURSELF
b. MAILING (normal post/registered post/post laju)
c. EMAIL (but all documents has to be in pdf form)
In my opinion, i PREFER to send the documents yourself to KKM.
It's the safest way as the staff can't claim they don't receive your
application.

To put your life easier, i recommend you to do it on the SAME DAY
you collect your Sijil Pendaftaran Sementara from MMC. You can
get your SIJIL PENDAFTARAN SEMENTARA CERTIFIED at
the PRIMARY SCHOOL just opposite MMC. The staff will ask you
to pay RM1 for the printing and certification.
